The Pandemonium Event turns three sets of Hell keys into organs and a final Uber Tristram encounter that rewards a Hellfire Torch.

WHERE & HOW TO FARM
Where to farm
Harrogath in Act V for Cube portal creation, followed by the randomly selected mini-Uber areas and Uber Tristram.
Treat this as a staged collection and boss sequence: gather keys, open three mini-Uber portals, collect their organs, then face Uber Tristram. Prepare for severe incoming damage and use the in-game portal results to track progress; the Torch is the event’s reward, but its class and stats vary.
Collect Keys of Terror, Hate, and Destruction on Hell difficulty.
Have enough survivability and damage for the mini-Ubers and final bosses.
Carry the Horadric Cube and keep keys and organs organized.
Farm three keys of each type from Countess, Summoner, and Nihlathak, counting only actual drops.
In Harrogath, transmute one key of each type to open a random mini-Uber portal.
Open the portals from separate key sets in the same game when possible, reducing the chance of duplicating a destination without access to all three organs.
Defeat the mini-Uber in each portal and collect its organ: brain, eye, or horn.
When all three organs are collected, transmute them in Harrogath to open Uber Tristram.
Defeat Uber Mephisto, Uber Diablo, and Uber Baal, then pick up and identify the Hellfire Torch.
Farming tips
Prioritize resistances, a Life Tap source, and Crushing Blow for the endgame fights; Magic Find does not help the key hunt.
Keep the portals and organs in one session so you can complete the chain without rebuilding intermediate progress.
Repeat, collect and use
Repeat with fresh key sets for another attempt; portals consume their ingredients, organs are collected from the mini-bosses, and one Torch is awarded per completed Uber Tristram run.
Common mistakes
Opening all portals from a single key set; each portal needs a full set of three keys.
Transmuting outside Harrogath; use the Act V town for portal creation.
Entering Uber Tristram without suitable defenses; prepare for the bosses’ damage and aura pressure before consuming the organs.
When to stop or switch
Stop after receiving a Torch that meets your character’s needs, or pause before the organ step if you need to improve gear for the final encounter.
